Doobert.com has been obtaining permission to use photo images of CVIs and health certificates as proof that the paper certificates were completed within 30 days. This way, the paper certificates do not need to be transported with the animals reducing the opportunity for them to be lost in the handover from one transporter to the next.

If an animal is being transported to an Animal Welfare Licensee in Illinois, the CVI must also include a permit number issued by the Department. The veterinarian completing the CVI may obtain a permit online at Online Animal Import Permit (also linked at the top of this page) or by calling 217-782-4944.
Dog Requirements
Certificate of Veterinary Inspection issued within 30 days of entry stating freedom from disease and vaccination against rabies for dogs > 4 months. Dogs may not originate from an area under rabies quarantine. Rabies vaccination must have been administered within the time period published in the current Compendium of Animal Rabies Vaccines prepared by the National Association of State Public Health Veterinarians, Inc.
Cat Requirements
No entry requirements. Cats are required to have a CVI if they are being transported to animal welfare licensees in Illinois.
